Important Information:
Appointments are only available online, and are held on Fridays, with initial appointments for 90 minutes and written treatment plan (cost: $369) and follow up appointments for 45 minutes (cost: $150). Please select carefully from ‘Tinnitus’ or ‘Hyperacusis’ (sound sensitivity), or ‘Tinnitus and Hyperacusis’. You will need access to a computer or tablet for the session.
After making your appointment you will receive a confirmation email with links to the appropriate forms and questionnaires depending on your selection. -Please complete these forms before your appointment so that I can prepare for our session together. -Please also email your hearing test and any other relevant reports (ie GP, ENT, ACC) to admin@tinnitusrelief.co.nz.

If your symptoms have been caused by an accident, you may be able to ask ACC to fund the consultation; however we must have prior approval from ACC for a TRT01 appointment with me at Tinnitus Relief Ltd (ACC Vendor Code G0K032), please contact your ACC Case Manager or Rehab team to request funding; we must receive a Purchase Order and approval from ACC prior to booking the appointment.
